Liability Waiver & Medicine Intake Form 

Given the sensitive and confidential nature of our work together in this Sacred container, there are two important processes you will need to complete prior to our first call together. The first is a Liability waiver which requires your signature. This document will be delivered to your email via Docusign. The second is your Medicine Intake questionnaire. The questions on this form will allow me to get a deeper sense of your personal history and set us up most powerfully for our work together.

Ancestral Breathwork Journeys 

As part of your medicine journey you will have the opportunity to participate in Ancestral Breathwork Journeys, which are scheduled on either a Tuesday or Thursday, as close to the New and Full moon dates each month. Through participating in these Ancestral Breathwork Journeys, you will be guided to tap into the power of your ancestral lineage and unlock profound levels of healing and growth.

The Breathwork Journey Calls will always take place from 6:00 PM to 7:30 PM PST, providing ample time for deep introspection and transformation. Below you will find links to integrate the schedule of Ancestral Breathwork Journeys into your personal calendar. Please be sure to check the calendar each month for specific details regarding the date and time of the Journeys. Each journey will have its own unique Zoom link for you to join, and you will be automatically registered for all of these journeys for the duration of your medicine program.

Prior to each Journey, we will send email reminders with the unique Zoom link. Therefore, it's essential to regularly check your email to stay up-to-date on the schedule and to access the link to join each journey.
